STATEMENT OF UNDERSTANDING FOR ATTENDING AN ACCREDITED PROGRAMME

An Officer has told me what an accredited programme is, and has given me a leaflet telling me about the programme that I am required to attend.

I understand that:

  • I must attend every session on time
  • I must not attend while drunk or under the influence of alcohol and drugs
  • I agree to take part in the session
  • If I miss any session, I will be required to complete a catch-up session before the next session
  • If I miss more than two sessions, I may have to restart the Programme at an approved point
  • If I am excluded from a session because of my behaviour or because I am late, then that counts as missing a session
  • If I am violent, racially abusive, sexually abusive or offensive to others then I will be barred from the programme and could be returned to Court and given another Sentence. If I am on licence, the Probation Service may recommend my recall to prison.
  • As part of selecting participants for the programme the Probation Service will interview me. This is to find out the best programme to use. I understand that as part of the long-term evaluation of the programme I will be asked to complete some questionnaires.
  • Each session of the programme will be recorded. This recording will focus mainly upon the staff rather than the participants and is used for monitoring and assessing staff performance. It may also be used for staff training. I understand that the recording will be stored securely and will only be disclosed where there is a legal obligation to do so, e.g. as evidence in court proceedings or to comply with subject access requirements under the data protection act.
  • If I am returned to Court, this form may be given to the Magistrates or Judge. If I am on licence, this form may be given to the Sentence Enforcement Unit or Prison Governor. This is to show that I understood the requirements of an accredited programme and that I agreed to take part.

The contents of this statement have been explained to me and I understand the requirements.
